Appointment Tips
Welcome to Arizona Advanced Surgery and thank you for choosing us to provide your care. Our goal is to make the registration process pleasant and convenient for our patients. Here are some tips to make the most of your first appointment with us.
- You can speed along your first visit by having your forms filled out prior to your visit. Upon making your appointment our friendly staff will guide you to an online registration link or they will mail you the paperwork depending on what is most convenient for you.
- Please bring with you any medical records pertinent to your condition. This will include X-rays, CAT scans, lab tests, or any other information regarding treatments from your primary care physician.
- Please also bring your insurance card and a photo ID.
- If your insurance plan requires a referral from your primary care physician, please have your doctor provide our office with this referral prior to your first visit.
- We request a 24 hour notice of cancelation for appointments, so that we may accommodate other patients.
